Create an ID from your first and last initials followed by your date of birth as month, day and year (MMDDYY). You'll enter this same ID on every survey, so the researchers never need your name.

Example: Jane Smith, born January 5, 1985 → JS010585

Your information is kept by TDF and used only to run this study and contact you about it. The research team receives your participant ID, not your name or contact details.
